Mechanism of action and indications: (Why med ordered) Blocking of calcium channels in blood vessels and myocardial cells, resulting in inhibition of excitationcontraction coupling and subsequent contraction. Systemic vasodilation results in decreased blood pressure. For Hypertension. Interactions with other patient drugs, OTC or herbal medicines (ask patient specifically): Also on Lisinopril- additive hypotensive effect.
Nursing Process- Assessment (Pre-administration assessment) Always check BP and pulse before giving. Assess for signs of CHF( peripheral edema, rales/crackles, dyspnea, wt gain, jugular vein distention.
Nursing Implications (what to focus on) Contraindications/warnings/interactions: Hypersensitivity; BP < 90 mm Hg systolic. Caution in history of CHF. Avoid grapefruit juice. Additive hypotensive effect with other antihypertensives. Common side effects: CNS: H/A, dizziness, fatigue. CV: peripheral edema, angina, bradycardia, hypotension, palpitations. GI: gingival hyperplasia, nausea. Derm: flushing. Lab value alterations caused by medicine: Monitor ECG periodically. Be sure to teach the patient the following about this medication: Avoid grapefruit juice. Instruct pt on correct way to monitor pulse if able. Change positions slowly. Maintain good oral hygiene, see dentist frequently to prevent gingival hyperplasia. Wear sunscreen to avoid photosensitivity reaction.
