ALL ENTRIES DUE BY OCTOBER 16 Please complete this form and follow all directions. Entries that do not follow these guidelines may not be considered for publication. All entries must be original compositions by the author listed. You may submit up to five works in all of our seven categories: fiction, non-fiction, poetry, photography, art, music, and film. Submit entry(s) in a large envelope with this sheet. Song and video submissions should include both lyric sheet and a CD/DVD of the item(s). Items will be returned upon request.
