I hereby certify that Donor, a non-profit organization established under the laws of Maryland, domiciled in Wheaton, has donated: Computers, Medical equipment and school supplies (see enclosed list) (enclose a list with the details of the donation) The beneficiary of the donation and consignee is: Name and Address of the Organization or Institute With regard to the above donation: 1.- The donation has no commercial value, and values provided in the shipping list are only for Customs purposes; 2.- The donated equipment and supplies have been donated solely for the ill, needy and children, and will be used in hospitals and clinics for low-income families; 3.- The donor has not received any goods or services in exchange for this donation.
