STUDY OF MATERNAL AND PERINATAL OUTCOME IN HELLP SYNDROME
Introduction: HELLP syndrome is a dangerous and life-threatening complication of severe pre- eclampsia. Incidence is 0.5-0.9% of all pregnancies and 10-20% in severe pre-eclampsia. Objective: Objective of this study is to know the incidence, maternal and perinatal mortality and morbidity of HELLP syndrome in severe pre-eclampsia and to diagnose atypical clinical features, so as to adopt timely intervention to salvage the mother and baby. Material and methodsIt is a retrospective study conducted in a tertiary care hospital in south India. Data taken from October 2015 to September 2018, which included 62 cases of HELLP syndrome out of total 302 cases of severe pre-eclampsia. Results: Incidence of HELLP syndrome was 20% in severe pre-eclampsia cases (8% partial and 12% complete). Primigravidae (common age <25 years) accounted for 50%. Abruption- 6.6%, oligoamnios- 20%, pathological CTG- 15%, eclampsia- 11%, most common in less than 28 weeks gestation- 46%, DIC- 8.3%, ARF- 3.3%, MODS- 3.3%, ICU admissions- 23%. Patients who received blood products was 36.6%, preterm delivery- 58%, NICU admission- 21.6%, IUFD- 3.3%, IUGR- 31.6%, MAS- 3.3%, fetal distress- 12%. There was no maternal mortality. Perinatal mortality rate was 14.43% Conclusion: This incidence of 20% in our study is high as our hospital is a tertiary care centre which receives referral cases – High risk with poor prognosis.Early detection and treatment can prevent most of the complications.
