Nutrition Department DECUBITI / PRESSURE ULCER INTERVENTION

PURPOSE:

To ensure patients with decubiti / pressure ulcers or patients at risk receive appropriate nutritional intervention in a timely manner to achieve the best possible outcome.

POLICY:

Nursing to assess patient for wound care. If patient at risk for or has wound/s nursing to notify by referral the clinical dietitian. A interdisciplinary approach for wound healing must be taken to ensure positive results. Nutritional Standing Order sheet to be completed within 24 hours of notification.

STEPS: 1.) Nursing to implement nursing intervention using standing order for wound care. 2.) PT to implement wound care intervention using standing orders. 3.) RD to implement wound care using standing orders based on clinical judgement. 4.) MD to review and sign off on standing orders on each patient. 5.) All disciplines to reassess as necessary and adjust patient care accordingly per clinical judgement.
