Regularly monitor physical as well as mental health of every child in the centre and promptly report any irregularity to promptly treat and cure the ailment.

CONDUCT OF DAILY HEALTH CHECK Every day, a trained staff member shall conduct a health check of each child. This health check shall be conducted as soon as possible after the child enters the child care facility and whenever a change occurs while that child is in Child Care Centre. The health check shall address: a) Changes in behaviour (such as lethargy or drowsiness) or appearance from behaviours observed during the previous day's attendance. b) Skin rashes, itchy skin, itchy scalp, or (during a lice outbreak) nits; c) If there is a change in the child's behaviour or appearance, elevated body temperature, determined by taking the child's temperature. d) Complaints of pain or of not feeling well. e) Other signs or symptoms of illness (such as drainage from eyes, vomiting, diarrhoea, and so on). f) Reported illness or injury in child or family members since last date of attendance. The facility shall gain information necessary to complete the daily health check by direct observation of the child, by querying the parent or legal guardian, and, where applicable, by conversation with the child. 3

Health Coordinator(HC): a staff member trained in monitoring the health of the children under his/her care. Nursing Assistant(NA): for paramedical support at the centre. Centre Head(CH): to coordinate the monitoring system and maintain contact with the parents/guardians to share information on child’s health. Medical Consultant (MC):for any emergency medical assistance needed for children during their stay at the centre. Daily Health Check at Child Care Centre

•HC conducts Grooming check in the presence of the child’s ward as soon as the child arrives at the centre. •HC to conduct Physical Check in the presence of NA. • HC to obtain medical attention for a child through CH. • HC to monitor through out the day every child under his/her care to check any irregularity in the child’s behavior. • HC to file reports endorsed by NA • HC to get doctor’s medical report from NA for any medical service provided at the centre to a child and mention it in medical report. • In emergency, HC toHealth inform a Child child’s guardian through the Daily Check at Care Centre Centre Head.

• CH to take daily update from HC • CH to organize through NA and MC medical attention for a child and update the child’s wards. • Organize discussions and meetings with the parents/guardians on children’s health. • CH to maintain a Child’s health record and monitor a child’s progress • Endorse through HC child’s health report based on information from HC, NA and MC. • Report to the child’s guardian the child’s health report. HC notify NA and CH any emergency.  CH to contact MC and the child’s guardians.  NA, HC, CH with MC provide immediate medical assistance to the child before entrusting him/her to respective guardian’s care.  HC to note all the details in the daily health report. (Report Template on slide 10)  NA to endorse the report.  HC to give the report to CH. 

HC prepare daily health check report of every child. (Refer Report template on slide 10)  NA endorse daily health check report.  HC submit report to CH.  HC discuss the report with a child’s guardian at the routine health meetings and discussions organized by CH. 
