1. Introduction 1. Objective: To develop an electronic health record for each and every citizen which is accessible on the network for the primary purpose of providing health care and health-related services. 2. Scope: It includes facilities like citizen can view his own health record, doctors can view his patients health record from birth, lab-technician and pharmacists can get their respective privileged views. 3. 2. Overall Description Citizens Health Record is a project, which aims to provide an Electronic Health Record to the citizens of India. In our country, the present system works without proper interoperability. That is, in some hospitals it may be in paper and where as in some other hospital it will be electronically recorded. But there is no system that ensures that a patient or a doctor can access these records at the same time in same format. Once the project is completed with all its functionalities, it will ensure that all the health records will be electronically recorded with interoperability and accessible to citizens and concerned health providers. The system also ensures that, it provide privileges to the doctors to view and modify the health record, lab-technician to modify the laboratory reports alone, the pharmacists can view the records concerning to the medication part and the citizens to view their health record.
