Case presentation : Aneurysmal bone cyst & Giant cell tumour Dr. jyoti
Aneurysmal bone cyst • Nonneoplastic solitary lesion of bone consisting of cystic cavity filled with blood .
• Seen in 5 – 20 yr of age Common in females
• Signs & symptoms : acute onset of pain at affected site with rapid increase in severity over a short period of time .
• Location : long tubular bone & spine ( 80 % ) , rarely in flat bones & short tubular bones in spine mainly seen in thoracolumbar region involving neural arches .
• Radiological features : rapidly growing expanding saccular lytic lesion in metadiaphyseal region On CT & MR show multiple fluid – fluid levels
Giant cell tumour • Neoplasm of bone arising from non bone forming connective tissue component of marrow
• Commonly seen in 20 – 40 yr of age (after fusion of epiphyseal plate)
• 80 % are benign & 20% are malignant
• Signs & symptoms : intermittent pain of aching nature with localised swelling and tenderness pathological fracture may occur
• Location : distal femur , proximal tibia , distal radius, proximal humerus o
sacrum is most common spinal site
• This tumour usually begins in metaphyseal region of long bone • in or adjacent to ossified epiphyseal line • extending to end of long bone abutting to joint surface leaving the lesion subarticular in location .
Radiological features • • • • •
Eccectric geographical lesion Meta-epiphyseal Subarticular Expanding with thining of cortex Internal septations giving soap bubble appearance
