Creamy Layer Certificate from Revenue Authorities for BC candidates as published in Para 1.13 of Commission’s Notification No.32/2007 in terms of Orders in G.O.Ms.No.3, BC Welfare (C-2) Department, Dated: 04-04-2006(State State Government Government proforma). proforma 5. Study Certificates from 4th Class to 10th Class for Regular Candidates and Residence Certificate for candidates who did not study in any Education Institution / Private study in 10th Class/Certificate of evidence to claim local candidate as per para 6 of Commission’s notification no.32/2007. Note:- In case the candidates who studied regularly in schools from

4th to 10th

classes have to submit study certificates obtained from schools(specimen proforma at page 43 of notification) . In case the candidates who did not study in any schools from 4th to 10th classes have to submit “ Residence Certificate” from MRO in the specimen proforma at page 44 of notification for 7 years period prior to 10th class. 6. Xerox Copy of Hall Ticket. 7. Xerox Copy of this Memo. 8. Filled in Check list.

2 9. Regular service Certificate issued by competent authority in case of candidates serving in AP State Government Service to claim age relaxation if any. 10. Discharge Certificate in case of claim as Ex-Service men obtained on or before 2812-2007 that is as on the date of issuing Notification. 11. Physically Handicapped Certificate clearly showing the percentage of deformity issued by the Medical Board. 2.
