Forms/documents should be submitted by the adviser after the School based Reading of Forms on March 14, 2019.  SF 1 ( School Register )  SF 2 ( Learner Daily Attendance Report)-For the month of February and March only) and Summary  SF 5 ( Report on Promotion and Level of Proficiency)  Form 138/Card  F137  Summary Grading Sheets with subject teacher’s initial  Certificate of Completion/Diploma ( Tentative date of Checking/Reading will be on March 18, 2019) Number of Copies to be submitted: SF 1 and 5 = 4 copies SF2 for the month of February and March -2 copies SF2 Summary of Class Attendance – 3 copies SF3 – 3 copies ( Date of Submission is on March 26, 2019) Summary Grading Sheets – 2 copies Note : School Forms (SF 1,SF 2 and SF 5 should be downloaded/Generated from LIS 4.
