AFFIDAVIT I, ELINA MORANDARTE CABAJOG-VICOY, Filipino, of legal age, married to David C. Vicoy, and a resident of Botoc Oriental, Loay, Bohol, Philippines, after having been sworn to in accordance with law, depose and say:

1. That I was born on August 16, 1964 in the Municipality of Compostela, Province of Davao to Spouses Ronauldo Cabajog and Teodora Morandarte; 2. That since I was a child my parents taught me that the spelling of my name was ELENA MORANDARTE CABAJOG. Hence, I used this name since I started schooling and even during the time that I worked with the public school;

3. That the schools where I studied did not strictly require the submission of authenticated birth certificate, for which reason I continued using the aforementioned name; 4. That on October 26, 1986, I took the Professional Board Examination for Teachers in Tagbilaran City, Bohol. At that time, I was not required to submit authenticated birth certificate from the National Statistics Office (NSO); hence, the name I used was still ELENA CABAJOG-VICOY;

5. That it was only when I applied for registration with the Social Security System in the year 1994 that I discovered that the spelling of my name as reflected in the Certificate of Live Birth from NSO is ELINA MORANDARTE CABAJOG and not ELENA MORANDARTE CABAJOG. When I discovered the error, I immediately processed the correction of my name in my school and employment records in order to reflect the correct spelling of my name; 6. That when I registered with the Professional Regulation Commission (PRC) in 1998, I used the name ELINA MORANDARTE CABAJOG-VICOY since my school records had already been corrected. However, I failed to petition for the correction of my name before the Office of the National Board for Teachers; hence, the report of rating and the certificate which I submitted to PRC during my first registration still bear my erroneously spelled name ELENA C. VICOY; 7. That on January 22, 1998, the PRC issued my license as a professional teacher bearing the correct spelling of my name ELINA C. VICOY. Because of this, I believed in good faith that my records in the said office had no problem especially of my first name; 8. That when I renewed my license in the year 2002, I discovered that my newly issued professional identification card reflected the wrong spelling

of my name ELENA C. VICOY. When I renewed the said ID in the year 2005, I filed a petition for correction of name at PRC Cebu; 9. That when I renewed the said ID in the year 2005, I filed a petition for correction of name at PRC Cebu; 10. That the petition in the year 2005 is the first and only petition that I filed with PRC; 11. That my report rating in the National Board for Teachers in the Professional Board Examination for Teachers held in Tagbilaran City on October 26, 1986 bears the name ELENA C. VICOY when it should have been corrected to ELINA C. VICOY as reflected in my Certificate of Live Birth. 12. That my report rating in the National Board for Teachers in the Professional Board Examination for Teachers shows my place of birth as NEW BATAAN DAVAO when it should have been corrected to PANAG, COMPOSTELA, DAVAO as reflected in my Certificate of Live Birth. 13. That I hereby affirm and confirm that the names ELINA MORANDARTE CABAJOG-VICOY and ELENA MORANDARTE CABAJOGVICOY refer to one and the same person; and 14. That I execute this affidavit to attest to the truth of the foregoing facts and to support my petition-letter to the Civil Service Commission (Cebu) for the correction of my name and place of birth in the Report of Rating issued by the National Board for Teachers to confirm with the name and place of birth appearing in my Certificate of Live Birth.
