Tutoring Confidentiality Statement

DIR®/Floortime™ San Diego Tutoring Confidentiality Statement DIR®/Flooritme ™ tutoring is a process in which participants present their work through written and video material and oral presentation, and participants and attendees engage in case discussion. All information about specific children, their families, professionals, and other people introduced in the discussion should be treated as confidential. Consents are obtained from the clients for release of this confidential information for the purposes of professional training. As a participant, I agree to treat all such information with professional confidentiality, and will not disclose any confidential information outside of tutoring sessions. In particular, if I have a personal knowledge about a client or any other person presented in a case discussion, I will refrain from divulging or discussing any information learned during the institute with that family or person/s, or any other person/s, without obtaining specific written consent to do so from both the presenter and the family. ________________________ Signature ________________________ Name (printed) ________________________ Date
