Golden Rule  “Consider

Guide for Basic Nursing Intervention  Review

the history (hx) of the patient (px).  Be equipped on the theoretical and technical aspects of the different areas of diseases (dx).  Continues update of knowledge through research.

A. Principles on Initial Contact with the Px.  Review

the history (hx) of the patient (px).  Review the px’s management and treatment. (Ex. Diet, v/s, s/p, dx, IVF and etc…)  Review, review and review…

B. Management and Intervention  Note

for the following:

 Physical

setup of the environment  Presence of significant relations to the px.  IV – volume, flow rate, number, main line and side drip (check for SD/ML incorporation/dilution) and IV location of each line. IV level equal to 50cc or BELOW should be changed to avoid lapse.  Drain tubes connected to the px.

Continued…  Perform

the review of system (ROS) and brief physical examination (PE) before and after px monitoring.  Utilize the General Health Status Assessment by obtaining subjective datas.  Be knowledgeable on the different process of Physical Examination and Neurologic Examination.

Continued…  If

there are any CHANGES, UNUSSUAL or UNTOWARD SIGNS in the px; notify and report it to the clinical instructor, staff nurse and physician (as the need arise).  If in Doubt; throw it OUT. Ask first and do not intervene.  ALWAYS HAVE THE KNOWLEDGE AND PRESENSE OF YOUR IMMEDIATE SUPERVISOR (CI’S) BEFORE INTERVENING.

and study all over again the case and documents of the px.  For better improvement and development of potentials through study: “Do not just be a VERY BEAUTIFUL DECORATIONS by being stagnant; sit down and relax but DO SOMETHING WHICH IS VERY PRODUCTIVE…being productive is priceless and appreciative.”

nursing diagnosis should be focused on the studied specialize area.  Always consider primarily independent intervention/action before dependent and collaborative.  To prioritize the problem that will be the basis for the diagnosis, should you consider the following: Maslows, 21 nursing problem, 14 BN and ABCD principle.

intervention and planning should be organized and systematized.  Not ALL intervention and planning are applicable, it’s a case to case basis.  Refer to NANDA

Diagnosis Conceptual Framework DX = Medical-Surgical, Maternal & Child, Community Health, etc…

Related to (r/t) = Anatomy and Physiology or related factor (RF NANDA)

Secondary to = etiology or the disease process Ex. Alteration in peripheral tissue perfusion r/t decreased peripheral blood flow (ischemia) secondary to Diabetes (disease process) as manifested by … (PE findings)
