SICKNESS REPORTING You must follow the sickness reporting procedure (Only in extreme circumstances do you arrange for someone to report sick on your behalf) : On your first day of sickness you should telephone your line manager and if not available please contact Business Support on (please note the office does not open until 9am) on 01563 553935 at least 2 hours before your session is due to start indicating: -
the nature of your illness how many days you are likely to be off when you expect to return to work any plans for obtaining medical advice the session(s) which require to be covered a contact telephone number
This information will then be passed to your line manager who will arrange cover for the session.
If your session takes place outwith office hours (Mon – Thurs 9am – 5pm and Fri 9am – 4pm) you should contact your line manager and provide them with the information highlighted above.
You must then telephone your line manager on the 4th and the 8th day of sickness updating on your progress. Saturday and Sundays count in sickness absences so if your first day of sickness is a Friday your fourth is a Monday. For absence of 7 calendar days or less a Sickness Report Form must be completed as soon as you return to work. The form can be obtained from your line manager. If you work outwith the office you must phone your line manger to confirm your return to work and they will carry out the Return to Work meeting by phone. .
For absence over seven calendar days a Statement of Fitness for Work (Med 3) must be submitted with the Sickness Report Form.
You must complete 26 weeks continuous service before you are entitled to sickness allowance from the Council.
