Request Form for Foreign Worker’s Occupation Code This form may take you 2 minutes to fill in.

Instructions: 1.

Please check for the occupation code via ‘Enquire Occupation Code’ in WP Online to make sure that the occupation code does not exist before you fax in the request form to Work Pass Division (WPD) at fax no: 6317 1386.

Please note that if the occupation stated is not found, we will assign the code closest to the main duties of worker given in the table below. The Work Permit application submitted using the occupation code will still be subjected to approval.

WPD will fax the form back to you within 2 working days from the date of receipt of the completed form.

Incomplete form will not be accepted.
