MARYLAND ZOO 6 GRADE FIELD TRIP PERMISSION SLIP th

May 18, 2009 Dear Parents/Guardians: On June 3, 2009, the 6th grade class will take a field trip to The Maryland Zoo in Baltimore, MD. The students and the 6th grade team are very excited about this valuable opportunity to extend our classroom studies of Math, Science, Social Studies, and Language Arts. We will be focusing, in particular on biomes and animal habitats. Students will need to bring a bag lunch to school or purchase a bag lunch from the cafeteria. We will depart and return during school hours. The cost of the field trip is $5.00 for students. Checks can be made out to Crestwood Middle School. If the cost of this trip causes a financial hardship on your family, please contact Debra Boyce directly at 240566-9016. Frederick County Public Schools makes every effort to assure student and staff safety on field trips. Please complete the form below and return it with money by Friday, May 22, 2009. Permission slip and money must be returned together to home base teachers. Students who do not submit a signed permission form will not be allowed to go on the field trip. Students who incur a referral that leads to a suspension after May 19 must bring a parent chaperone for the trip.
