How To Complete an Electronic Session Feedback Form     This new electronic session feedback form is replacing the paper version of the Teaching Mentor Comment Sheet. You will still need to complete this form after each mentoring session. This feedback will help inform the Assistant Center Director on how your session went as well as ensure that Higher Achievement is tracking the skills that our Scholars obtain through their mentoring sessions. Review this short guide to understand how to complete this form through our information management system, ISIS. 1. Log in to ISIS. 2. On the left navigation, click on My Schedule. The screen below will display; this is where you will begin to complete the session feedback form.

3. Indicate which of your Scholars were present during this session by placing a check in the box next to their names. 4. Click the Session Feedback Form button.

5. It is important to choose the correct date that you had your mentoring session. The system defaults to the present day, so use the dropdown menus to indicate the actual day you completed the mentoring session. 6. The open box in Question 1 is the area for you to indicate any Scholars that joined your group that night that you are not officially assigned to. 7. Complete the remaining fields on the page.

8 8. When finished, click the Submit button at the bottom of the page. 9. After you click Submit, your Assistant Center Director will be notified that you completed the form. After he or she reviews it, they will be able to respond to any of your questions and concerns. You will receive an e-mail with a summary of your comments and his or her responses. Feel free to continue to have conversations at Center discussing how to better challenge your Scholars.
