DAMAGE CERTIFICATE
We certified that on dated 31/08/09 – Under our transport receipt no. 505 DT. 31/08/09. We have loaded 2915 boxes of ceramic tiles from SHREEJI CERAMIC INDUSTRIES, Lalpar via Morbi. For m/s RADHA MARBLE, BALLABGARH (HARIYANA) – As per Invoice no. 00938. In Truck (trailor) no. HR 47C 4018. Unfortunately said truck meet accident near Kishangarh Dist. Ajmer on 2/9/09 – Vehicle overturned and tiles destroyed/badly damaged. We agreed the damages as per survey report for Rs. 1, 37, 000/- one lac thirty seven thousand only... Issued without Prejudice morbi DT. 12/11/09.
