A complainant in case for “RECKLESS IMPRUDENCE RESULTING TO DAMAGE TO PROPERTY, SERIOUS PHYSICAL INJURIES AND A CASE FOR PERJURY against Edgardo Ortega and Jonathan Ortega.

4. Please state your reason why are you executing this affidavit? I am executing this affidavit to attest to the truth of the foregoing facts and for whatever purposes it may serve. 5. What was the incident all about on that said time, place and date? On October 22, 2017, my mother Celerina A. Casing and I were travelling with my father. We were riding a motorcycle with plate number LC20685 number with my father Jose Casing as our driver. He was driving the motorcycle with due care at a regular speed.

6. What happen next, if any?

On our way to the said place, a vehicle Hyundai Porter Railings overtakes us. As the Hyundai driver was overtaking us, there is another vehicle coming from the opposite direction travelling at a fast speed. In order to avoid an impending collision, the driver of the Hyundai vehicle swerves to our lane hitting us unfortunately. 7. What was your condition after the Incident? I was shocked, felt some pain and cannot sleep.My mother was hospitalized. She was brought to Bukidnon Provincial Hospital – Maramag.
