Annexure – 3 You are required to submit the following documents within a week of your acceptance. Education (All photocopies should be clear) Photocopy of Degree Certificate (Both Sides) Graduates
Photocopy of all 3 years marks cards Photocopy of provisional degre certificate (both sides)
School
10th / 12th std Marks card or II PUC marks card Current Passport Primary Documents Minimum of 1 document required.(with photo identity)
Election ID Card Driving License Family Card PAN Card
Identity Verification (All photocopies should be clear)
Latest Bank Statement / Passbook / Debit card with photo Ration Card (self) Secondary Documents (in case Primary is not available). Minimum of 2 required
Date of Birth certificate or transfer certificate Marriage Certificate 10th Marks card will be accepted with photograph LL with Photograph 10th Marks card will be accepted in cases where that is not his highest education
Note: all employee must have 1 primary ID doc , if no primary available (10th & LL(with photo) to be produced) Checks conducted for all employments held in the last 5 years prior to Mphasis EDS - for Tigers & Uoperate(last 7 years mandatory) for Indigo(all employments) Employment (All photocopies should be clear)
Primary Documents (either 1 will be required)
Photocopy of Relieving letter Photocopy of Experience letter Salary slips (for 1 month minimum)
Secondary Documents
F&F Paper Resignation acceptance letter signed and reflecting employee number
You are required to report to the HR department on your day of joining. Contact Person Address Reporting Time
