Admitting Impression: a.)Acute Renal failure 2nd to Diabetes Mellitus

Action Taken: a.)Examined and was diagnosed of myocardial infraction or (heart attack) b.) Immediate treatment of the said condition was carried out and less than one hour there after the patient expired.

Cause of Death: a.)Cardio pulmonary arrest secondary myocardial infraction or (heart attack) secondary to diabetes mellitus

4.)

Issued upon the request of the party concerned and fro whatever legal purpose it may deem served.
