Chantilly, VA Grade Field Trip

Dear Parents/Guardians: On December 16th and 17th 2009, the eighth grade students will take a field trip to the Udvar-Hazy National Air and Space Museum in Chantilly, VA. The students and staff are very excited about this valuable opportunity to extend our classroom studies and enhance our knowledge in science. Due to the size of our 8th grade class, half of the students will attend on December 16th and half will attend on December 17th. Frederick County Public Schools make every effort to assure student and staff safety on field trips. In order to keep all students safe, we are requiring that all students return the permission form below. Students are asked to pay a small amount toward the cost of this trip to help cover the cost of transportation. Please complete the form below and return it along with a payment of $3.00 by Wednesday, December 9. Checks may be made out to Crestwood Middle School. Students who do not submit a signed permission form and payment will not be allowed to go on the field trip. Late permission slips will not be accepted. All students planning to attend must hand in forms to their home base teacher by Dec. 9. If you have any questions please do not hesitate to call Mr. Julia at (240) 5669107. Students may pack a lunch, bring money to buy a lunch at the McDonald’s in the museum, or have the school cafeteria pack a lunch for your child. Please check the box at the bottom of the slip if you would like the cafeteria to provide a lunch on the day of the field trip. Crestwood Middle School is not responsible for damage to, or loss of, any items students bring on the field trip. Students who misbehave on this trip will be disciplined accordingly and may lose the privilege to participate in other eighth grade field trips and events planned for the school-year. We are looking for parents/guardians who are willing to chaperone this trip. Please check the appropriate box below if you are able to chaperone. Cut along the line below. Keep the above portion.
