That I am a Secondary School Teacher 1 by Profession with Professional Regulation Commission (PRC) License I.D.; 2. That I always had the PRC I.D. inside my bag; 3. That sometimes, on December 2017, I discovered said I.D was no longer in my bag and I believe that the same was misplaced when I was out shopping in town; 4. That I exerted diligent efforts to locate it but to no avail, hence, I hereby consider it lost for all legal intents and purposes. 5. That I am executing this affidavit to attest to the veracity of the foregoing statements and for the issuance of a new one in lieu of the loss.
