Torres Age: 17 years old Sex: Male Civil status: Single Religion: Roman Catholic Date of admission: July 05, 2009 Room no.: 3rd Floor Surgical Ward no.2 – NGH Complaint(s): Right lumbar pain Impression: a) Markedly enlarged left kidney with thinned corticles and severe left hydronephrosis due to ureteropelvic/ proximal ureteral stricture. b) Right medullary nephrocalcinosis Physician: Dr. Vivina C. Family History and Individual Information, Social and Health History. Be on the lookout for data which has bearing on present health condition. Patient first knew about the urethral stricture when he was 9 years old, he noted abdominal pain wherein the UTZ of the whole abdomen taken revealed cystic mass on the left hemi abdomen and CT revealed left hydronephrosis and was advised surgery but patient opted to go for a second opinion which was similar to this admission. Patient reported to be fond of eating junk foods, drinking softdrinks, smokes 4 sticks of cigarette a day and drinks alcoholic beverages occasionally.

Level of growth and development Patient weighs 44 kg and measures 160 cm tall. Studying at a night highschool near their residence in Consolacion, Cebu as a first year highschool student. Patient eats variety of foods, 3 small meals with snacks in between. He has no difficulty in micturation but defecates every 4 days.
